If you need your security code for the Patient Portal, please call our office. Upon completing your Patient Portal information, please print and bring the completed Patient Forms with you to your appointment.

If you are unable to complete all of the forms prior to your appointment, it is required that you arrive 30 minutes before your appointment time.

Cancelation Policy:

We ask that all appointments be confirmed within 24 hours of the scheduled appointment time. Any patient who no-shows or cancels within the 24 hours will be charged a cancelation fee - $50 for medical visits and $100 for cosmetic visits.
