What to bring to your first appointment

• New patient forms: Financial Policy, HIP A A Private Practice Acknowledgement, Privacy Policy, New Patient Health History
• Insurance card and photo ID
• Medications: Either bring a complete list of all medications you take, or bring all of the bottles
• Name, address, and telephone number of your preferred pharmacy
• If you have specific questions or issues you wish to discuss with the doctor, it may be helpful to make a list so you don’t forget anything

Verification of insurance

We highly recommend sending a copy of your insurance card to our office in advance, as this will make the verification process go much more smoothly. You can send it by fax or email (). Be sure to include both sides of the card.

If your insurance coverage cannot be verified and you do not have your insurance card, you will be classified as a self-pay patient unless and until insurance is verified. In that case, your credit card will be charged at the time of your visit. However, the charge is reversible if insurance is resolved within two days.
