About Your Visit
We thank you for choosing Healthy Woman as your healthcare provider. We will make every effort to make your experience as easy as possible.We have a 24-hour cancellation policy
When you come in for your appointment, you will be asked to sign several forms, including registration forms, HIPAA required forms, and other office policy forms authorizing us to submit billing information to your insurance company. Each patient will also be asked to provide a urine sample at every visit. So, please come prepared by drinking adequate amounts of clear liquids prior to your visit.
We ask that you bring the following to each of your appointments:
- Current Insurance Card
- Co-Pay (if applicable). we accept VISA, MasterCard, American Express, cash, and personal checks
- Referral from Primary Care Physician (if applicable)
- Records from your prior physician (if you are transferring to our practice and have not already had your records sent to us)
Which forms should I print and complete?
For your convenience, we have all of the necessary forms available for you on our website. In order to expedite the registration process, we ask that you please print, complete, and bring the forms with you when you visit.
If you cannot print these forms at home, they will be provided to you upon check-in. You can

Patient Rights
As our patient, you have many rights and responsibilities. We understand that you want to develop a respectful relationship between you and your health care provider. We encourage patients to speak with us about any concerns they may have. All of our patients have the following rights:- To receive respectful treatment
- To enjoy confidentiality of their health records
- To enjoy confidentiality and privacy with the exception of life threatening situations
- To receive information regarding participation in research and be allowed the option to refuse participation without compromising medical treatment and care
- To ask for alternatives to certain treatments
- To receive the opinion of another professional in regards to the health and treatment of a specific condition
- To participate in their health care
- To have accessible information regarding diagnostics and services available
- To be informed of legal reporting requirements
- To have the ability to file a complaint and/or appeals
- To have family involvement in the care and treatment of specific conditions
